Tirzepatide: The Dual Agonist Behind the Biggest Shift in Weight Management

Tirzepatide is the compound that proved a single molecule could do two jobs at once and get better results than either alone. What tirzepatide is

Tirzepatide (originally coded LY3298176, developed by Eli Lilly) is a once-weekly dual agonist. It engages two receptors that govern how the body handles food and fat: GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1). Targeting both with one molecule is what gives it an edge over the single-receptor compounds that came before it.

What the research shows

The landmark trial is SURMOUNT-1, published by Jastreboff and colleagues in the New England Journal of Medicine in 2022. It enrolled 2,539 adults with obesity or overweight and ran for 72 weeks. Participants on the highest dose (15 mg) lost an average of 22.5% of their body weight, and about nine out of ten participants taking tirzepatide lost weight. Just as notable was the quality of that loss: participants achieved roughly a three-times greater reduction in fat mass than lean mass (33.9% fat versus 10.9% lean), which means the weight coming off was overwhelmingly fat.

Tirzepatide’s track record runs deeper than a single study. The SURPASS program established its effects on blood sugar control in people with type 2 diabetes, and follow-up analyses of SURMOUNT-1 have reported benefits for blood pressure and overall metabolic health. This is one of the most thoroughly studied metabolic compounds available.

How it works

The two receptors complement each other. GLP-1 curbs appetite and slows gastric emptying, so people feel full faster and for longer. GIP adds a second mechanism, strengthening the insulin response and improving how the body processes and stores fat. Hitting both at once produces a stronger, smoother effect on appetite and metabolism than targeting GLP-1 alone, which is the core reason tirzepatide outperformed the single-agonist generation before it.
