Cagrilintide takes a different road to the same destination as the GLP-1 compounds, and that difference is exactly what makes it valuable. It is a long-acting amylin analog that works on appetite and fullness through a separate pathway, which means it adds to GLP-1 results rather than overlapping with them. Paired with semaglutide, it has produced some of the strongest weight-loss numbers in obesity research.

What cagrilintide is
Cagrilintide is a synthetic version of amylin, a hormone the body releases with insulin after eating that signals fullness and tells you to stop. Cagrilintide is engineered to be long-acting, with a half-life of roughly 180 hours, which makes it suitable for once-weekly dosing. Because it works through the amylin pathway rather than the GLP-1 pathway, it brings a fresh mechanism to the table.
What the research shows
Cagrilintide proved itself first on its own. A dose-finding phase 2 trial by Lau and colleagues, published in The Lancet in 2021, tested once-weekly cagrilintide in 706 adults with obesity or overweight and established meaningful weight loss as a standalone compound.
Its biggest moment came in combination. Cagrilintide paired with semaglutide is known as CagriSema, and the phase 3 REDEFINE 1 trial, published in the New England Journal of Medicine in 2025, enrolled 3,417 adults and reported an average weight reduction of 20.4% at 68 weeks, compared with 3.0% for placebo. Sixty percent of participants lost at least 20% of their body weight, and nearly a quarter lost 30% or more, placing it among the most effective weight-loss results recorded to date. Earlier proof-of-concept work in The Lancet (Enebo and colleagues, 2021) had already shown that adding cagrilintide to semaglutide produced additive weight loss beyond what semaglutide achieved alone.
How it works
Amylin and GLP-1 both reduce appetite, but they do it through different signalling systems, and that is the whole point. Cagrilintide activates amylin receptors to promote satiety and slow gastric emptying, dampening hunger through a route that GLP-1 compounds do not touch. Because the two pathways are distinct, combining cagrilintide with a GLP-1 agonist produces an additive effect: each one suppresses appetite in its own way, and the body responds to both at once. That is why CagriSema outperforms either component on its own.

What to look for when buying
